Indications:
Aspirin inhibits platelet aggregation, so the indications of aspirin enteric-coated tablets are as follows:
Reduce the risk of acute myocardial infarction in suspected patients
Prevention of recurrence of myocardial infarction
Secondary prevention of stroke
Reduce the risk of transient ischemic attack (TIA) and its secondary stroke
Reduce the risk of patients with stable and unstable angina
After arterial surgery or interventional surgery, such as percutaneous transluminal coronary angioplasty (PTCA), coronary artery bypass (CABG), carotid endarterectomy, arteriovenous shunt
Prevention of deep vein thrombosis and pulmonary embolism after major surgery
Reduce the risk of myocardial infarction in patients with cardiovascular risk factors (family history of coronary heart disease, diabetes, dyslipidemia, hypertension, obesity, smoking history, and people over 50 years old).
Adverse reactions:
The listed adverse drug reactions (ADRs) are based on the spontaneous post-marketing reports of all aspirin preparations (including short-term and long-term oral treatment), so there is no need to organize according to the frequency category of CIOMS III.
Upper and lower gastrointestinal tract discomfort, such as indigestion, gastrointestinal and abdominal pain. Rare gastrointestinal inflammation and gastroduodenal ulcer. It is very rare that gastrointestinal bleeding and perforation may occur, accompanied by laboratory abnormalities and clinical symptoms.
Aspirin may increase the risk of bleeding due to its inhibiting effect on platelets. The observed bleeding includes bleeding during operation, hematoma, epistaxis, urogenital bleeding, and gingival bleeding. There are also reports of rare to extremely rare bleeding, such as gastrointestinal bleeding, cerebral hemorrhage (hypertensive patients with poor blood pressure control and/or combined with anticoagulants), which may be life-threatening. Acute or chronic bleeding may lead to anemia/iron deficiency anemia (such as occult microbleeding), accompanied by laboratory abnormalities and clinical symptoms, such as weakness, pallor, hypotension.
Patients with severe glucose-6-phosphate dehydrogenase (G6PD) deficiency have hemolytic and hemolytic anemia.
Renal injury and acute renal failure.
Allergic reactions are accompanied by corresponding laboratory abnormalities and clinical symptoms, including asthma symptoms and mild to moderate skin reactions.
Respiratory tract, gastrointestinal tract and cardiovascular system, including rash, urticaria, edema, pruritus, cardiovascular respiratory system discomfort, and extremely rare severe reactions, including anaphylactic shock.
Very rare transient liver damage with elevated liver transaminase.
Dizziness and tinnitus have been reported when the drug was overdosed.
Taboo:
Aspirin enteric-coated tablets are prohibited under the following conditions:
·Allergy to aspirin or other salicylates, or any other ingredients of drugs;
·History of asthma caused by salicylates or substances containing salicylates and non-steroidal anti-inflammatory drugs;
·Active peptic ulcer;
·Hemorrhagic constitution;
·Severe renal failure;
·Severe liver failure;
·Severe cardiac failure;
·Combined with methotrexate (dose of 15mg/week or more) (see drug interaction);
·The last three months of pregnancy.
Matters needing attention:
Aspirin should be used with caution when:
·Allergy to analgesics, anti-inflammatory drugs, anti-rheumatic drugs, or other allergic reactions.
·History of gastroduodenal ulcer, including chronic ulcer, recurrent ulcer and gastrointestinal bleeding.
·Combined with anticoagulants (see drug interactions).
·For patients with impaired renal function or cardiovascular circulation (for example, renal vascular disease, congestive heart failure, hypovolemia, major surgery, sepsis or severe hemorrhagic events), acetylsalicylic acid may further increase the risk of renal damage and acute renal failure.
·For patients with severe glucose-6-verate dehydrogenase (G6PD) deficiency, acetylsalicylic acid may induce hemolytic anemia in hemolytic adults. Factors that can increase the risk of hemolysis, such as high dose, fever and acute infection.
·Liver function damage;
·Ibuprofen may interfere with the effect of aspirin enteric-coated tablets. If patients take aspirin and ibuprofen together, they should consult a doctor.
·Aspirin may cause bronchospasm and cause asthma attacks or other allergic reactions. Risk factors include bronchial asthma, hay fever, nasal polyps, or chronic respiratory infections. This also applies to patients with allergic reactions to other substances (such as skin reactions, itching, rubella).
·Because aspirin can inhibit platelet aggregation for several days, it may lead to increased bleeding during or after surgery.
·Low dose aspirin can reduce the elimination of uric acid and induce gout.
Pharmacology and toxicology:
Aspirin inhibits the production of thromboxane A2 and thus the aggregation of platelets. Its mechanism is irreversible inhibition of cyclooxygenase synthesis; Because these enzymes in platelets cannot be synthesized again, this inhibition is particularly significant. Aspirin has other inhibitory effects on platelets. Therefore, it can be widely used in cardiovascular diseases.
Aspirin is an acidic non-steroidal drug with antipyretic, analgesic and anti-inflammatory properties. Usually 0.3-1g aspirin is taken orally to relieve pain and low fever, such as cold and flu, which can reduce body temperature and relieve joint muscle pain.
Aspirin can also be used for chronic and acute inflammation, such as rheumatoid arthritis, osteoarthritis and stiff spondylitis. Generally, 4-8g of high dose can be taken every day to treat these diseases.
Pre-clinical safety data
Animal experiments showed that high dose salicylic acid caused renal damage but no other organ damage. In vivo and in vitro tests showed that aspirin had no teratogenicity and carcinogenicity. Salicylates may have teratogenicity in different kinds of animals. It has also been reported that taking it during pregnancy can affect the implantation of pregnant eggs, cause embryotoxicity and fetal toxicity, and hinder the learning ability of offspring.
