Product Description
Product name: Ribavirin InjectionMolecular formula: C8H12N4O5Molecular weight: 244.21Character: This product is a clear, colorless liquid.Indications: Antiviral. Used for viral pneumonia and bronchitis caused by respiratory syncytial virus.
Usage and Dosage:Dilute with sodium chloride injection or 5% glucose injection to a solution containing 1mg per 1ml and then drip slowly intravenously.
Adults 0.5g (5 sticks) once, 2 times a day, children 10~15mg/kg a day according to body weight, divided into 2 doses. Each drip for more than 20 minutes, the course of treatment for 3-7 days.
Adverse reactions:Common adverse reactions include anemia and weakness, which disappear after discontinuation of the drug. Less common adverse reactions include fatigue, headache, insomnia, loss of appetite, nausea, vomiting, etc., and may cause a decrease in red blood cells, white blood cells and hemoglobin.
Taboo: It is prohibited for those who are allergic to this product and pregnant women.Matters needing attention:1. Use with caution if you have severe anemia or abnormal liver function.
2. Interference with diagnosis: Oral administration of this product can cause an increase in blood bilirubin up to 25%. Large doses may cause a decrease in hemoglobin.
3. Early use of the drug. Respiratory syncytial virus pneumonia is generally effective when given within 3 days of the beginning of the disease. This product should not be used in patients without laboratory confirmation of respiratory syncytial virus infection.
4. Long-term or high-dose administration has adverse effects on liver function and blood picture.
Pregnant women and nursing mothers:
1. This product has strong teratogenic effect, the daily dose of 1mg/kg in rabbits can cause embryonic damage, so it is prohibited for pregnant women and women who may be pregnant (the elimination of this product in the body is very slow, 4 weeks after stopping the drug is not completely cleared from the body).
2. A small amount of the drug is excreted in breast milk and is toxic to both mother and child, so breastfeeding should be suspended and the milk discarded during the drug administration. This product should not be used in lactating women due to the self-limiting nature of respiratory syncytial virus infections in such cases.Pharmacology and toxicology:Pharmacology: Broad-spectrum antiviral agent. It inhibits the growth of respiratory syncytial virus, influenza virus, hepatitis A virus, adenovirus and other viruses in vitro by a mechanism that is not fully understood. It does not alter viral adsorption, invasion and decapitation, nor does it induce the production of interferon. The drug is rapidly phosphorylated upon entry into virus-infected cells, and its products act as competitive inhibitors of viral synthase, inhibiting inosine monophosphate dehydrogenase, influenza virus RNA polymutase and mRNA guanosyltransferase, thereby causing a decrease in intracellular guanosine triphosphate, impairing viral RNA and protein synthesis, and inhibiting virus replication and propagation. It may also have immune effect and neutralizing antibody effect on respiratory syncytial virus.
Toxicology: Animal studies have found that this product can induce benign tumors in the breast, pancreas, pituitary and adrenal glands, but its carcinogenicity in humans is not confirmed. The drug can cause malformations of the head, palate, eyes, jaws, bones and gastrointestinal tract and reduced offspring viability in hamsters and other animals, but experiments in primates did not reveal the effect of the drug on fetuses. Cardiac damage occurred in mice, rats and monkeys given oral ribavirin at doses of 30, 36 and 120 mg/kg or for more than 4 weeks (human equivalent: 4.8, 12.3 and 111.4 mg/kg for children weighing 5 kg or 2.5, 5.1 and 40 mg/kg for adults weighing 60 kg).
Drug Interaction:
This product has antagonistic effects when used with zidovudine, as it inhibits the conversion of zidovudine to the active form of zidovudine phosphate.Overdose:
Large doses can cause heart damage, and in patients with respiratory diseases (chronic obstructive pulmonary disease or asthma) can cause breathing difficulties and chest pain.Pharmacokinetics:
The plasma concentration is (17.8±5.5) mM after 5 minutes and (42.3±10.4) mmol after 30 minutes of intravenous infusion of 0.8 g. The drug is rapidly distributed to all parts of the body and can cross the blood-cerebrospinal fluid barrier. The concentration of the drug in respiratory secretions is mostly higher than the blood concentration. The drug can enter the red blood cells and accumulate in large amounts. The concentration of the drug in the cerebrospinal fluid can be 67% of the blood concentration in the same period after long-term use. The drug can pass through the placenta and can also enter the breast milk. It is hardly bound to plasma proteins. It is metabolized in the liver. Plasma drug elimination half-life (t1/2β) is about 0.5~2 hours. Mainly excreted by the kidney, (16.7±10.3%) of the drug can be detected in the urine in its original form and (6.2±1.7%) of the drug is excreted as metabolites within 48 hours. The drug can accumulate in the red blood cells for several weeks.Storage: Airtight storage.
